Enrollment Process:
Adults with a myriad diagnoses (Alzheimer’s disease or other dementias, multiple sclerosis, strokes, Parkinson’s disease, young adult with special needs and many more) come to spend their days with us.
Some participants are unable to stay home alone due to health and safety concerns. Some participants like to stay busy or looking for excitement in participating in activities. That’s what Tender Loving Care Adult Day Health Center is- safe and loving place devoted to comfort and care.
What is the recommendation process?
1. Introductory inquiry Phone Call
2. Initial Tour of TLC( by appointments)
3. Formal Assessment( by appointment)
When someone calls in to inquire about services, we have a very natural and simple conversation over the phone. We can usually tell by the conversation if the potential Guest sounds like he or she might be a good fit at Tender Loving Care ADHC and if we might be a good fit for their needs and desires. Families are always welcome to stop by and take a tour. We offer a free Guest day without any obligation. If there is interest in services, we invite the family to visit Tender Loving Care Adult Day Health Center and participate in a formal assessment. And if Tender Loving Care ADHC isn’t the right fit, we do everything in our power to make sure that they receive the power care from another venue.
Our formal criteria are fairly simple. The participant must be 18 years old and older. Also, must be able transfer. We accept participants who need assistance with activity of daily living.
What should I bring to the assessment?
• Physical examination paperwork ( TLC can provide in advance)
• Legal paperwork, including PDA
